Meet with you, your loved one and other involved family to assess the current situation, identify what might be needed, make recommendations and help put a plan in place, of your choosing, to meet your needs. The meeting can be at home, a hospital, nursing home or other location.
Work closely with the Elder Law attorneys and paralegals at PLG to ensure that your legal and financial plans are fully coordinated with your care plan.
Advocate to ensure that you get what you want and need in order to maintain maximum independence and quality of life for your loved one and involved family.
Arrange an array of in-home services ranging from home maintenance to home health aides and transportation services.
Help you transition a lived one to an assisted living facility or nursing home.
Monitor your loved one’s health care status and supportive services for families at a distance. You can know that a professional is overseeing your loved one’s care and checking back with you when change is necessary. We hope you will also value the caring relations hip you enjoy with your care manager, someone who is available whenever you need assistance.
Your GCM is an objective third party. In trying times, families don’t always see eye-to-eye. A trained professional and skilled facilitator, your GCM may be just what the doctor ordered!
Care Management Services
Assessment – Review current situation to identify problems, eligibility for assistance and need for services.
Assistance – help with health insurance claims and hospital discharge planning.
Placement – assist with moving a older person to or from a retirement/adult home or a skilled nursing facility.
Advocacy – Be the liaison with entitlement programs, institutional facilities and insurance companies and identify appropriate community resources.
In-home Services – Arrange various in-home help, from visiting nurses to housecleaning; monitor services, suggesting changes as necessary.
Support – offer counsel to older adults and/or involved caregivers/family members.
Mediation – Assist in reconciling a family’s concerns and opinions that may diverge in times of crisis.
